This survey template is intended to provide Toronto employers with a way to collect feedback from employees who may begin returning to their workplaces in the coming months. The City of Toronto hopes to ensure that employees continue to, or resume commuting in a safe and sustainable manner. No single option will work for all, so it’s important to understand the unique experience of each employee.

We’re giving you 12 questions you can use for your own workplace survey. These will provide invaluable insights to help you craft a strategy that will help employees perform at their best. Learn how remote work impacts productivity, how many days a week your employees want to be at home, and more.
